1. PurposeAMP Healthcare Education maintains permanent records of all continuing education activities to ensure accurate documentation of learner participation, CEUs issued, and protection of participant privacy.
AMP Healthcare Education retains primary responsibility for the maintenance, security, and availability of all permanent learner records, including records stored on contracted digital platforms.
2. Permanent Educational Records
A permanent record is the official cumulative record documenting an individual learner's participation in AMP Healthcare Education continuing education activities. For each learner who successfully completes an activity, the permanent record includes:
- Provider name and address
- Learner name and contact information
- Activity title
- Type of activity (e.g., online course, self-study, live workshop, webinar)
- Start and completion date(s)
- Number and type of CEUs awarded (where applicable)
- Instructor(s) name(s) and credentials
- Assessment scores, where applicable
- All CEUs issued are tracked alongside completion records as part of the learner's cumulative file.
Retention: Permanent records are maintained for a minimum of seven (7) years from the date of activity completion and serve as the official record of participation.
3. Record Storage Systems
Online Courses
For online and self-paced activities delivered through the LearnWorlds platform, learner records are securely maintained within the platform. Records include enrollment details, participation, completion status, CEUs awarded (if applicable), and issued certificates.
Live Events & Workshops
For live educational activities, AMP Healthcare Education maintains secure records within a restricted-access Google Drive environment. These records include attendance verification, completion dates, CEUs awarded (if applicable), instructor details, assessments (if applicable), and copies of issued certificates.
Access to all systems is restricted to authorized personnel.
4. Availability of Records
Certificates of completion containing any relevant CEUs issued are available to learners upon request within the seven (7) year retention period. These records serve as the official permanent documentation of participation.
5. Identity Verification & Release of Information
When replacement records are requested, learner identity is verified using identifying information on file (e.g., full name, email address, course details). Educational records are released only:
- Directly to the learner, or
- To a third party with documented written consent from the learner.
- No information is disclosed without explicit learner authorization except where required by law.
6. Privacy & Data Protection
AMP Healthcare Education:
Collects only information necessary for course administration and CEU documentation
Maintains records within secure digital systems
Restricts access to authorized personnel
Does not sell or share learner information except as required by law or with learner consent
7. Complaints Record
AMP Healthcare Education maintains a secure record of all complaints filed against the provider, including both resolved and unresolved complaints, for a minimum of seven (7) years.
